MINOR (CHILD) PHOTO RELEASE
I, the parent/legal guardian of the camper named above grant City Church Lloyd my permission to use photographs for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content. Furthermore, I understand no royalty, fee, or other compensation shall become payable to me by reason of such use.

MEDICAL CONSENT AGREEMENT
I, the parent/legal guardian named above authorize the Ministry Personnel of City Church Lloyd to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant (camper) named above. I undertake and agree to indemnify and hold blameless the Ministry Personnel, City Church Llloyd, it's pastors and board from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of City Church Lloyd as well as any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when participating in or travelling to events of the City Church Lloyd.
